5) a contact number which you can be contacted  ;

6) the preferred mode of delivery  :

•     Self collection at Serangoon MRT station 
•     $5 delivery charge at desired MRT station 
•     $10 for delivery to Singapore address 
•     FREE delivery in Singapore for purchases above $100
